These name cards are to be worn by students in clinic to identify you as Bachelor of Optometry students. If you lose your card you must request a replacement immediately.

• These cards are only to be worn by current BOptom students.
• The replacement fee of $20 is non-negotiable and non-refundable.

You will be contacted by the GSA at SOVS reception, when your campus card is ready for collection.
